For those engaged in research involving animals at UCD the following is required:

1. All personnel involved in animal research must be named on a current IACUC approved protocol


2. Personnel Qualifications (PQ) Form: Please submit with new, or rewrite, protocol submissions and with the addition of new associates. Please submit to the IACUC office F489


3. Occupational Health Enrollment: Please arrange a consultation with the Animal Program Occupational Healthcare Provider, or your own health care provider. Please submit all Occupational Health Forms to F-489 or fax to 303-724-0989


4. Attendance at an orientation session
