For those engaged in research involving animals at UCD the following is required:

1. All personnel involved in animal research must be named on a current IACUC approved protocol


2. Personnel Qualifications (PQ) Form: Please submit with new, or rewrite, protocol submissions and with the addition of new associates. Please submit to the IACUC office F489


3. Occupational Health Enrollment: Please arrange a consultation with the Animal Program Occupational Healthcare Provider, or your own health care provider (using the standardized form). Annual renewals are required for all levels except aquatics. Please submit all Occupational Health Forms to F-489 or fax to 303-724-0989


4. Attendance at an orientation session
