All personnel who are listed on research
protocols, as well as some ancillary employees such as Facilities or Security
personnel, must be enrolled in the UCD Occupational Health Program.
Personnel involved in animal experiments
must submit a Personnel Qualifications Form to the IACUC Office and be named
on a current IACUC approved protocol.
Each individual has the option of
utilizing the UCD contract occupational healthcare provider or the healthcare
provider of their choosing. The benefits of using the UCD contract provider
are that all forms will be processed for you and the provider is familiar with
the particular health risks you face when working with research animals.
If you see your own provider, you must take the UCD standardized enrollment
form with you and then fax the form to 303-724-0989.
Requirements of the initial enrollment
vary based upon the species to be handled. Individuals working with rodents
must have a minimum of a current tetanus inoculation and an allergy risk assessment.
In addition to the initial screen for the Occupational Health Program,
each individual must complete an annual update. Occupational Health Forms should
be submitted to F-489 or via fax at 303-724-0989.
