Personnel involved in animal experiments
must submit a Personnel Qualifications Form to the IACUC Office and be named
on a current IACUC approved protocol.
Each individual has the option of
utilizing the UC Denver occupational healthcare provider or the healthcare provider
of their choosing. The benefits of using the UC Denver provider are that
all forms will be processed for you and the provider is familiar with the particular
health risks you face when working with research animals. If you wish
to consult your own provider, please contact the UC Denver occupational health
healthcare provider at 303-724-4663 for advice on the forms required.
Requirements of the initial enrollment
vary based upon the species to be handled. Individuals working with rodents
must have a minimum of a current tetanus inoculation and an allergy risk assessment.
In addition to the initial screen
for the Occupational Health Program, each individual must complete an annual
update. All Occupational Health Forms should be submitted to F489 or via fax
at 303-724-0989.
