At enrollment, 28-to-32 weeks gestation, and at 6-month intervals during the first two postpartum years CAMP patient completed a series of pre-coded, self-administered, multiple choice questionnaires. These surveys are both clinical and research tools. They are written at a fourth grade reading level and collect information about program participants’ sexual and reproductive histories and the social context of their pregnancies. Particular emphasis is placed on identifying treatable demographic, psychosocial, and behavioral risk factors for preterm and low birth weight delivery, inconsistent contraceptive use and repeat teen pregnancy, and child abuse and neglect. Responses are used to develop differential diagnoses and treatment plans to prevent these untoward outcomes. In addition, to standardize the collection of clinical information and the recording of research data, referral and encounter forms have been developed for the health care providers, case managers, social worker, and dietician. Finally, optimize the measurement of broad constructs, such as depression and relationship dynamics, well validated, psychometrically sound scales are available. |
