CME Program Planning Document/Sponsorship Application

Submit the application via campus mail or U.S. mail with original signatures and the required accompanying needs assessment documentation.

We will not review hand written or incomplete applications.   Please make sure all applications are complete with needs assessment documentation, a budget and the appropriate signatures.

Upon acceptance of your application a deposit will be requested.  Work on your activity will start once the deposit is received.
