What You Can Expect From Your Visit to the Senior's Clinic
First visit:
- Scheduled for one hour with physician
- May also be scheduled to see geriatric nurse practitioner
- Memory loss evaluations include assessment by nurse practitioner
and social worker
- Discussion of major health concerns
- In-depth social, functional and medication review
- Preventive health assessment (immunizatuibsm exercise, recommended
screening tests)
- History and physical
- Treatment goals & options are determined
- Advance directives discussed and/or information provided
- Social worker will see patient and/or family members for
information and problem-solving
Follow-up visits:
- Scheduled visits
- Episodic or urgent care (clinic RN provides phone triage)
- Physician or nurse practitioner
- Patient education (nurse practitioner, pharmacist) for specific
health concerns

- Diabetes care
- Anticoagulation ("Coumadin Clinic")
- Managing asthma and chronic lung disease
- Referrals to specialists as needed
- Referrals to other members of the Senior Clinic team:

Information to bring at your first visit:
- Completed form
which willbe mailed to you before your first visit
- All of your medications (prescription, over-the-counter, herbs, vitamins)
- Copies of advance directives (Living Will, Durable Power of Attorney for Health Care)
- Pertinent old medical records
- Insurance information
Eligibility: all persons age 65 and older
with Medicare A/B. We do not currently participate in Medicare
managed care contracts.
Hours: Monday - Friday 8:00 am to 5:00 pm, with evening and
