2008-2009

Click Here for a PRINTABLE PDF version of this page.

STEP 1

Free Application for Federal Student Aid (FAFSA)

Complete the FAFSA for the appropriate academic year using your Federal Student Aid PIN*.  Be sure to list UCD Anschutz Medical Campus (school code 004508) on the FAFSA.  After the FAFSA is processed, you will receive a Student Aid Report (SAR) from the FAFSA processor. Review it for accuracy and additional requirements. If it states that more documents are required, provide them to our office. (See step 2).

*Students and parents must have their own Federal Student Aid PIN in order to sign the FAFSA electronically.  If you need a PIN, apply for one at www.pin.ed.gov.

STEP 2

University Application for Financial Aid (UAPP)

and Verification Worksheet

Complete the appropriate UAPP/Verification Worksheet based on the academic year and your dependency status.  Sign and date all forms in ink, and FAX them 303-735-6113.  Please remember to include a signed copy of your 2007 federal tax returns and any other documents indicated on the Verification Worksheet.  Be sure your name and student ID (or SSN) are on the top of ALL documents submitted.  Keep copies of all documents for your records.

Additional Documentation You will be notified, by mail, if any additional documents such as U.S. citizenship or immigration documents, Selective Services registration confirmation, and/or social security card are required. Requests for additional documentation will be mailed to the address on the student database.  Please keep this information current and respond promptly to all requests.  If you are unsure whether more paperwork is required, or if you have questions, contact the Financial Aid Office.

STEP 3

The Award Offer

It will take approximately 4 – 6 weeks to process your award from the date your file is completed.  Once your file has been completed and reviewed, you will receive an Academic Year Award Offer in the mail.  Please read and follow all instructions on the back of your award offer.  Students will automatically be awarded any grants and loans that they are eligible to receive.  Maximum grants will be awarded before any loan is offered. You may refuse and/or reduce your loan amounts without penalty.

STEP 4

Information on choosing a lender and completing your promissory note(s) and other loan documents will be included on your Academic Year Award Offer.
