Center for Health Promotion

The Center for Health Promotion at the University of Colorado uses advanced information technology to:

  • track patients through the process of determining eligibility for outreach for primary and secondary preventive services
  • generate letters and postcards inviting patients to discuss and arrange for receipt of these services by phone
  • log telephone contacts
  • record referral/scheduling activities
  • posts test results
  • generate reports for the electronic medical record

The system is designed to facilitate receipt of guideline-based therapy for:

  • diabetes mellitus
  • hypertension
  • cardiovascular disease
  • cancer screening
  • immunizations
  • bone densitometry
  • other types of primary and chronic disease services

In addition to improving quality and access in primary care, the system supports improvements in pay-for-performance benchmarks, accurate and up-to-date disease registries, patient satisfaction and convenience, and it provides powerful opportunities for health care organizations to market a highly unique service and establish competitive contracts with medical payers.
