Program Agreement Letters

Program Agreement Letters (formerly known as Educational Letters of Agreement) are required for all rotations.

The template for Program Agreement Letters includes all required information.


Instructions for Completing the Template

  1. Download the template by clicking on the link.
  2. A window will pop-up asking what you would like to do with the file.
  3. Confirm that "Save this file to disk" is selected (this is the default) and click OK.
  4. A new window will pop-up asking you where to save the file.
  5. Select the location on your hard drive where you would like the file to be saved.
  6. A window will announce when the download is complete.
  7. Open the file using MS Word and complete the template by replacing the bold sentences with actual information.
  8. Once the template is completed send to Alisha Konig in the Office of GME for review.
  9. Once all signatures are obtained, send a copy to the Office of GME to be maintained in binder file.
  10. This agreement needs to be updated at least every five years but as soon as there is a change in any of the individuals who signed the agreement.
