BEFORE
YOU GO, YOU SHOULD KNOW:
As
you leave your UCDenver School of Medicine GME training program, you
are required to complete the exit procedure. This applies to everyone,
including residents who will become faculty. Reading this information
will save you time.
- DO
NOT CLOSE YOUR BANK ACCOUNT
until your last paycheck has cleared. Final paychecks will be direct
deposited. If you have additional money due after your last regular
paycheck, i.e. internal moonlighting, this will also be direct deposited.
To avoid delay in receiving your W2, keep your address updated with
Payroll and Benefit Services (303-735-6500, Option #2).

ALL
INSURANCE BENEFITS END at MIDNIGHT
(unless completing a shift) on the ending date of your current
CU GME training agreement. You have no "grace period."
(If you are not sure of the correct ending date on your agreement,
contact your residency/fellowship program coordinator to verify.)
Health insurance ends.

Certification of Group Health Plan
Coverage Letters regarding your CU GME Health &
Dental coverage termination will be mailed to your home address
from EBMS. This letter will likely be required by your next
insurance plan. Please keep it. You will need it. If you
think you did not receive this information, or have misplaced
it, call EBMS Eligibility (1-800-777-3575 ext 1272) for instructions
on how to obtain a copy.

Group
long term disability coverage ends. Guaranteed-issue
conversion to individual disability policy is available. Click
here for additional
CU GME Disability coverage information.

Life
insurance ends. Conversion of your current group
life insurance is available through SYMETRA Life Insurance Company.
Generally, the conversion is only to your advantage if you are
"uninsurable" (i.e. have a pre-existing condition).
To obtain the forms necessary to request conversion rates, contact
GME Benefits at 303-724-6024.
- Immunization/PPD
records
are
not maintained with GME and are not accessible at or through the
GME Office. You should have your own documentation of these records.
You will need it through out your professional career. If you
do not have the documentation you previously provided, you may
email KJL Healthcare Management, klee80111@gmail.com,
to request a copy of your information. Phone requests are not
permissible. Your email serves as authorization to provide this
information to you. The information will be provided to the resident
only (no third parties). Residents who started CU GME training
PRIOR to 5/2001 and need immunization records should contact UCH
Medical Records Release of Information at 720-848-6214 or 720-848-6253
for instructions.

request
proof
of malpractice coverage
call 303-724-6031 or email
the GME office. Proof of coverage will be provided to the resident
only (no third parties) via mail, fax or
pick-up by the resident at the GME office, Research Bridge Room
1401. (Specify in your request how you want sent.) When
applying for a COLORADO license, GME will send directly to the
Colorado Board of Medical Examiners, as well as send a copy to
your home address for your records.
Request for claims history must come directly from, and is provided
directly to, the requesting entity.GME
does not release your claims history directly to you.
- Requests
for verification of training
should
be sent directly to the office of your training program
director. (GME can verify dates of training, but cannot provide
evaluation information.) Give your director's correct address,
including campus box number, when completing forms for privileges,
etc.
- GME
PARKING PRIVILEGES
end
on the last day of your training. If
you still have a Parking Access Card/Permit, please return it
to the Parking Office at Anschutz Medical Campus, Building 500,
Ground Level - across from Information Desk. Parking access through
GME will be turned off. Information from Parking and Transportation
on unpaid citations will be mailed directly to your home address.

GME PAGER
use ends. Turn in your pager, all ID badges and all other University-issued
property to your coordinator, along with your Medical Records
& Campus Offices Clearance Form, on your last day of training.
Your pager will be turned off.
Residents
are NOT eligible for unemployment
compensation.
You will
receive a form for MEDICAL RECORDS & CAMPUS
OFFICE CLEARANCE from your residency coordinator on or about
June 1, so please be on the look out for it. You are required to obtain
a sign-off signature from the health information management (medical
records) department of each facility (except UCH) where you saw patients
during this academic year. On your last day, your final stop will be
with your program coordinator, where you will return any university-issued
property, e.g. pager, keys, lab coats, UCD ID badge, etc., and submit
your clearance form so your coordinator can complete your check out.
IF
YOU ARE BECOMING A CU FACULTY MEMBER:
Your insurance benefits through GME end. You must enroll
in a faculty health insurance plan in order to receive benefits.
Contact your new department/division administrator now to find
out how to enroll in a faculty benefits plan. If you need to
purchase the GME health insurance extension for interim time,
see the above information on the CU GME Health Benefits Plan
Extension.
GME
Parking Privileges end on the last day of your training.
Contact the parking office at 303-724-2555 to arrange for a
new card.
GME
Pager use ends. Your pager will be turned off, and must
be turned in to your coordinator. If it is decided you are to
retain a specific pager number, contact your department/division
administrator to arrange. (The pager funding must be transferred
to a different source. The GME source will be discontinued as
of your last day of training.)
