9. What is required to be included in program training manuals?

10. How does a resident obtain a copy of immunization records?

Immunization/PPD records are not maintained with the GME office. Residents should have a copy of these records as the resident provided them when s/he entered a GME training program and should have kept a copy of PPD documentation each year when provided to GME's designated provider, KJL Healthcare Management.
If the resident did not retain a copy of documentation, s/he may email KJL Healthcare Management at klee80111@gmail.com * to request a copy of the information. Phone requests are not permissible. Email will serve as the resident's consent for release of records. Records will be provided only to the resident (no third parties) via return email. The resident should keep a copy of these records and can then provide directly to requesting entities.
*This is a new email address for KJL HealthCare effective 1/1/2009.
