1. What are the current and upcoming salaries (stipends)?

Salaries for each PGY level are posted at http://www.uchsc.edu/gme/stipends.htm and are posted for the next year at the end of March.

2. Am I required to pay FICA?

Medical residents are not entitled to the student exception for FICA, according to the IRS Office of Chief Counsel (Tax Exempt and Governmental Entities). The IRS has concluded that generally, unless the facts are virtually identical to those in the Eighth Circuit case (University of Minnesota where residents pay tuition), services provided by medical residents are not incident to and for the purpose of pursuing a course of study, but rather a form of on-the-job training, and therefore not entitled to the statutory exception from FICA.

3. How do I change my payroll deductions?

Print a W-4 form from www.cusys.edu/pbs/forms/. Complete the form and submit it to Payroll & Benefits Services, 3100 Marine St. 6th Floor; Boulder, CO 80303 or intercampus mail: A006/Box 575. Call Payroll at 303-735-6500, option 2, with questions regarding your W-4 form.

4. I changed banks. How do I change/correct my direct deposit information?

Print a Direct Deposit Application form from www.cusys.edu/pbs/forms/. Complete the form, attach a voided savings deposit slip (when requesting deposit be made to a savings account) or voided check (for deposit to a checking account) and submit all information to Payroll and Benefits Services, 3100 Marine St. 6th Floor; Boulder, CO 80303 or intercampus mail A006/Box 575. Call Payroll at 303-735-6500, option 2, with questions regarding your direct deposit information.

5. My parking card is not working. What do I do?

6. My pager is broken, lost, etc. What do I do?

Go to Information Systems Service Center, located in Building 500 Anschutz Medical Campus, 4th floor. or call (303) 724-4357.

7. My stamper is lost, stolen, broken, etc. What do I do?

Replacements for lost, stolen or broken stampers will be at the Resident's expense. Approximate charge will be $11. Replacement stamper is obtained by contacting: Beth Hessel-Kirkman, Corporate Express, 303-373-8207 with the following information:

1. Identify yourself as a Resident with the University of Colorado School of Medicine.
2. You are requesting a 2000 plus pocket stamp.
3. Provide your first and last name.
4. Mailing address where stamper can be sent.
5. VISA/Mastercard number.

8. It would be nice if there was a contact list of for all my other problem solving issues. Does that exist?

9. How do I obtain a copy of my immunization records?

Immunization/PPD records are not maintained with the GME office. You should have your own copy of these records that you provided when you entered a GME training program and also have kept a copy of PPD documentation each year when you provide it to GME's designated provider, KJL Healthcare Management.

If you did not retain a copy of your documentation, you may email KJL Healthcare Management at klee80111@gmail.com * to request a copy of the information. Phone requests are not permissible. Email will serve as your consent for release of records. Records will be provided only to you (no third parties) via return email. You should keep a copy of these records and can then provide directly to requesting entities.
(Residents who started CU GME training PRIOR to 5/2001 and need immunization records should contact UCH Medical Records Release of Information at 720-848-6214 or 720-848-6253 for instructions.)
