In order for residents to have access to any of the affiliated hospitals computer terminals, dictate charts, etc, they must complete a security form and be entered into the HISDI database.
Program Coordinators are resonsible to enter residents into the HISDI database regularly. To enter information into the database go to www.uchsc.edu/hisdi
Before you can enter information you need to obtain a login ID and password from Gail Silber. She can be reached at 303-724-6025.

When does the IS Security form have to be completed and handed in?
Any
resident who will be rotating to either University of Colorado Hospital,
the Children's Hospital, Denver Health, Rose Medical Center, Presbyterian/St.
Lukes and/or Swedish at some time during the year, please have him/her
complete an IS Security form for each hospital. In addition the program
coordinator must enter demographic information into the HISDI database
found at www.uchsc.edu/hisdi.
