New Program Request Form

Faculty who are interested in establishing a GME clinical training program, fellowship or residency, must complete the Request for New Program Form and present information about the program to the Graduate Medical Education Committee for approval. Approval allows Departments to process trainees under the GME, which allows for particular benefits.

Guidelines for Establishing a New Program

  • Programs must be at least 6 months in duration
  • There must be an apparent need and rationale for establishing a new program
  • There must be a well-defined administrative structure
  • There must be a written program curriculum
  • If accreditation is available, the Program Director should seek accreditation either
    by ACGME or any other entity
  • There must be clear source of funding for training costs

Process for Obtaining Approval

  1. Program Director contacts GME Office Staff, (Jean Hart, PhD) to inform of intent to establish a new clinical program
  2. Program Director completes the Request for New Program Form and submits to Office of GME. Download the form by clicking on the link. The can be opened and completed using Microsoft Word.
    Program Director presents the reasoning for establish a new Program to Graduate Medical Education Committee during one of its meetings.
  3. If the program can be accredited by the ACGME, in addition to the request for New Program form, the faculty must complete the ACGME application for accreditation or Program Information Form (PIF) and submitted to Office of GME along with Request for new Program. The PIF can be obtained from the ACGME website at www.acgme.org
  4. GMEC makes a decision right after director presents the rationale for establishing the program.
  5. Office of GME staff officially notifies the program director of the Committee's decision within 2 weeks after the meeting.

Instructions for Completing the Request for New Program Form

  1. A window will pop-up asking what you would like to do with the file
  2. Confirm that "Save this file to disk" is selected (this is the default) and click OK
  3. A new window will pop-up asking you where to save the file
  4. Select the location on your hard drive where you would like the file to be saved
  5. A window will announce when the download is complete
  6. Open the file using MS Word and complete
  7. Move through the spaces using the tab key
  8. Print form and obtain appropriate signatures
  9. Submit the form to Office of GME- Box C-293
