The following information has been provided to assist students in completing their graduation paperwork in a timely fashion. Please review all the materials for the appropriate packet. Students should pay special attention to the instructions and deadlines. Deadline dates are not flexible. Missing a deadline date will result in rescheduling your exam or moving your graduation date to the next graduation cycle.

**If you took courses at the UCDAMC as a non-degree student and prior to your matriculation into the Graduate School, they must be transferred into your graduate program curriculum to be used to fulfill degree requirements. If you have not already transferred these courses, you will need this form. This is also the form to use to transfer in coursework taken outside the CU system.

To complete the MS Graduation packet, you also need to complete a diploma card. It may be completed on-line at the following website: http://www.uchsc.edu/registrar/diplapp.php. The Registrar's Office has deadlines for submission of this card. Notification of these deadlines is emailed to students each graduation cycle.

Bookbinding - The University will submit your thesis to Denver Bookbinding Company for binding for the library. You are free to use any binding company you wish. For more information contact http://www.DenverBook.com . The pricing on the web page does not reflect an average of 20% discount given to UCDAMC students. When placing your order, just request HSC pricing. The discount is on the binding pricing only, not on the extra charge pricing.
