Medicare Post-Acute Care: Quality Measurement
(Post-Acute Care Quality Measures)

In this study we will begin by selecting four tracer conditions that involve different patterns of post- acute care including skilled nursing facilities, home health agencies, rehabilitation hospitals/units, and outpatient settings. Based on an extensive literature review and consensus among clinical experts, we will select quality measures, data items, and risk adjusters for quality measurement across post-acute providers. Emphasis will be placed on measures applicable to post-acute care in fee-for-service and HMO Medicare systems.
