Evaluation of the Effects of Case Management for High-Risk Elderly Patients

Project Period: 2/96 - 8/98
Using data gathered in the Rehabilitation Alternative Study from five HMOs, we
will analyze discharge planning surveys gathered from case managers and
social workers. Among the various issues studied will be a comparison of
those services planned at the time of discharge with those services actually
provided in the ensuing months.
