A Randomized Trial to Improve Transitions Between Healthcare Settings for Persons with Chronic Illness
(Care Transitions)

This project has two primary aims: 1) To develop a new patient-centered measure designed to assess the quality of care transitions, the Care Transitions Measure (CTM) and 2) To develop and test a new patient-centered intervention designed to enhance the capacity of the patient/caregiver unit to participate and manage their care across different health care settings. Intervention patients will be compared to similar controls derived from administrative data.
