Health Information Exchange in Post-Acute and Long-Term Care

Goals: The purpose of this project is to describe the current status of the use of health information
technology (HIT) in health delivery systems, identify the factors that support or deter the timely
exchange of needed health information, identify policy levers that could be used to encourage
information exchange, and summarize and organize information learned and describe the next steps
that could be pursued to extend health information technology into post-acute and long-term care.
