The Role of Care Coordination in Reducing Emergency Department Utilization

The purpose of this study is to examine the association between use of emergency services and poor care coordination in a managed care setting. The study design is a nested case-control of older managed care patients with complex care needs. Comparisons will be made between 125 cases and 250 controls. Care coordination is measured both from the perspective of the individual patients through administering a telephone survey of a validated instrument and from the perspective of the relevant providers through review of chart data and utilization data.
