• Print, complete, and bring in the New Patient Packet
• Obtain an insurance authorization/referral as needed
• Remember to bring:
  • Health Insurance card(s)
  • Infusion logs
  • Pertinent medical records
  • Names, addresses & phone numbers of your Primary Care Physician and dentist
  • A pair of shorts for physical therapy examination
  • Pediatric Patients: Immunization Records
