Clinical History:

A 37-year-old black male was treated sporadically for 3 years for a chronic condition. He developed an upper respiratory tract infection and collapsed suddenly. Paramedics found him unconscious in ventricular fibrillation. Resuscitation was not successful. A photograph of his 520 gm heart and an H&E stained section of his left ventricular myocardium (with a normal for comparison) are shown in Figure 1 and Figure 2 respectively.
