Clinical History:

A 55 year old diabetic male became increasingly short of breath with a feeling of lightheadedness and vague chest pain over the period of 2 days. His family took him to the emergency room.

Questions:

1. What is your differential diagnosis?

Based on the examination and preliminary tests he was admitted and therapy started.

2. What tests would have been ordered based on the differential diagnoses ? 

3. Figure 1 is a gross photograph corresponding to his diagnosis and shows two cross sections side by side of the left ventricle. What does it show? and what is the most likely cause of this finding?

The next day a mitral valve regurgitant murmur was heard.

4. What is this most likely due to?

On the 7th day he had difficulty moving his left hand and lower extremity and examination revealed muscle weakness.

5. What is the possible cause of this?

On the 8th day he suddenly deteriorated with acute severe hypotension and cardiac arrest. He could not be resuscitated.

6. What is your differential diagnosis?

7. Figure 4 is a gross photograph with a cross section of the heart. What does it show?
