Clinical History:  An 18 year old male, previously in excellent health experienced a grand mal seizure. Physical exam, including a careful testicular exam was normal. CT scan of the head revealed multifocal defects consistent with metastases. Biopsy of an enlarged right supra-clavicular node was performed.

Questions:

1. In Figure E1 - Photomicrograph of the sections taken from the lymph node biopsy. Describe the two large cell types that are present. What is your diagnosis?

2. What serum test should have been ordered before the patient's orchiectomy? What should the result have been?

On follow up examination, a small enlargement of the patient's right testicle was noted and an orchiectomy was performed:

3. Figure E2 shows a photomicrograph from the patient's testicular mass. What is the diagnosis?

4. Figure E3 shows an immunohistochemical stain for beta-HCG performed on the same specimen and is seen in Figure E2.  What is the significance of this finding?
