Overview
The University of Colorado Denver sponsors a fellowship in pediatric emergency medicine (PEM) through the Section of Emergency Medicine at The Children's Hospital. This pediatric subspecialty-residency-training program was accredited by the ACGME (Accreditation Council of Graduate Medical Education) in 1999 and re-accredited in 2004. The subspecialty-residency-training program offers a four-pronged curriculum, including training in clinical pediatric emergency medicine, research, teaching and administration.
Clinical Training Program
The clinical training program is conducted such that the standard of care is maintained for patients and families while providing the appropriate environment for PEM subspecialty residents to learn, take responsibility and become skilled clinicians. The faculty attending physicians must sustain close involvement with patient care, including patient evaluation, subspecialty resident supervision and case-to-case teaching, while promoting subspecialty resident individual growth. The fundamentals of assessment, generation of a differential diagnosis, problem-solving, prioritizing, preliminary management plan formation, follow through of plans, ongoing evaluation and reassessment, final diagnosis and disposition with meticulous attention to discharge instructions, follow-up and return instructions with documentation of all of the above is emphasized consistently and regularly. The clinical experience at the primary institution, The Children's Hospital, involves evaluation and care of children aged twenty-one years of age and less, with the full spectrum of medical, surgical, psychiatric, gynecologic and major and minor traumatic injury and illness. The Children's Hospital is a Level 1 Regional Trauma Center with American College of Surgeons accreditation.
The goal of this pediatric emergency medicine subspecialty residency (PEMSR) is to produce individuals who are experts in the practice of clinical pediatric emergency medicine, which includes the assessment and management of the acutely ill or injured child. Subspecialty residents will become adept at and experts in PEM technical skills as well as in the coordination and management of patients receiving critical care transport. Specific areas of clinical knowledge and expertise are acquired through rotations during a three-year curriculum. Rotations include pediatric emergency medicine, adult emergency medicine, anesthesia, sports medicine and orthopedics, toxicology, pediatric critical care, prehospital care and specialty rotations such as radiology, plastic surgery and child advocacy and protection.
Research Curriculum
The research training program is designed to provide subspecialty residents with the education, and resources required to achieve the following goals: All subspecialty residents undergo formal course work in biostatistics, epidemiology and clinical research design. Twice-monthly research conferences allow subspecialty residents the opportunity to present research study ideas and receive valuable feedback prior to initiating a research project. All subspecialty residents choose a research mentor to further aid them in the process. There is emphasis placed on critical review of medical literature through journal clubs and departmental and regional meetings. All subspecialty resident trainees must design and conduct a prospective clinical study and are required to submit and gain approval for an IRB proposal and present in either an oral or poster form at a regional or national scientific meeting.
Teaching Curriculum
Pediatric emergency medicine subspecialty residents must be expert in teaching one-on-one at the bedside. They must be experienced and competent in formal and informal lecture preparation and presentation. They must be effective public speakers, teachers of resuscitation skills, teachers of procedural skills and experienced in outreach programs to local, state and regional audiences. All subspecialty residents are PALS instructors and are actively involved in resuscitation teaching through the Mock Resuscitation Program. Through these teaching experiences, subspecialty residents are directly responsible for educating pediatric, emergency medicine and family medicine residents, nursing and ancillary staff, as well as prehospital personnel. Through active participation in journal club, case conference and other Department of Pediatrics conferences such as morning report and noon conference, the subspecialty residents are responsible for the education of their peers, attending physicians and themselves. The pediatric emergency subspecialty residents must also be effective subspecialty consultants for the community and region.
Administrative Curriculum
Our subspecialty residents are trained to be experts at Emergency Department supervision and they are provided experience in quality improvement, risk management and Emergency Department leadership. They are also exposed to marketing, financial management and reimbursement issues.
Academic Career
Other areas included in the training are guidance for the development of an academic career and vision for an individual future, a departmental future and the future of pediatric emergency medicine as a field.
