SAN LUIS VALLEY COMMUNITY HEALTH SURVEY A Community Health Survey is coming to the town of Saguache and southern Conejos County. The survey will be conducted in the town of Saguache beginning the week of August 10, 2009 and in southern Conejos County beginning early September. The purpose of the survey is to identify adult health needs of the community and provide local data for community groups to plan ways to address these needs. The survey is being conducted by the Rocky Mountain Prevention Research Center (RMPRC) of the School of Public Health at the University of Colorado Denver. The RMPRC has been in the SLV for the past 25 years conducting studies on various health concerns including diabetes and cardiovascular disease. The local office of the RMPRC is located in Alamosa. Over the years, the University of Colorado/RMPRC has partnered with many SLV community organizations and members to plan and carry out research important to the community. The survey has been planned with input from many organizations and individuals in the local communities. The RMPRC has met with the Saguache Town Council, a local health support group, and representatives of city and county administration. Partners in this survey include Alamosa County Public Health / Nursing Services, Valley-Wide Health Systems, Inc., the San Luis Valley Comprehensive Community Mental Health Center, the SLV Regional Medical Center and the SLV GIS / GPS Authority. There will be upcoming meetings with community members in southern Conejos County. Staff members have and will continue to attend community events and distribute flyers announcing the survey. The health survey will be conducted by telephone and in-person. Members of the staff of the RMPRC trained in survey data collection will call residents or visit them at their homes. Data collectors will leave a telephone message or door hanger when no one answers the telephone or door. Adult residents will be randomly selected to answer the survey. The goal is to collect at least 100 surveys per community. Participation is important in order to obtain a representative sample of residents. Data collectors may be recognized by the yellow shirts they will be wearing. They will have proper identification displayed. The Alamosa County Survey is an ongoing, population-based telephone survey being conducted in the San Luis Valley to assess health behaviors, attitudes, and concerns, and to compare these to state and national trends in these behaviors, attitudes, and concerns.
