RADIOACTIVE MATERIALS WASTE PICK-UP REQUEST FORM

Instructions:

1. Complete all applicable fields.  Please be as specific as possible.
2. List the quantity of each item of waste, isotope, and activity  (e.g., 2 H-3 0.25 mCi, 3 P-32 1 mCi, 2 S-35 0.5 mCi)
3. Send the form to HSD by clicking on the submit button below.
4. HSD will contact you within the next three business days in order to schedule a date for a waste pickup.
5. If you need an emergency pickup please call 303/724-0345 and request to speak with the individual performing the radioactive waste pickups.
6. We are unable to pick up your waste unless you have a current laboratory survey available for review.
7. For mixed waste, please call 315-7962 or 724-0345.
