Disposal of Surplus Laboratory Equipment, Computers, and Furniture
("Green-Tagging")
To request equipment disposal, click on the appropriate form below, fill it out, and submit it electronically to your Department Head for approval. Your Department Head then forwards the form to Space and Asset Management using the embedded e-mail links in each form.
In the case of Medical/Scientific/Lab equipment, the form is sent to both Space & Asset Management and EH&S as indicated at the bottom of that form. If you have any questions about submitting these forms, you can contact Space and Asset Management at 303/315-2257.
