Online Laboratory Registration Form UCD is required to identify the location of every room or research space assigned to Principal Investigators. Researchers must complete this form for every laboratory space at the AMC prior to generating any hazardous chemical wastes. Provide the names of three (if possible) knowledgeable researchers (emergency contacts) who are familiar with operations of the research laboratory.
