On-line Registration
For online registration, complete the Registration Form and submit electronically. Payment, however, must be made separately, either by check or by credit card. For checks, make check payable to UCHSC and mail to address below. For credit cards, print and complete Credit Card Payment Form and mail to address below or fax to (303) 724-0911.

Mail-in Registration
Print and fill out the Registration Form. For payment by check, mail registration form and check payable to UCHSC to address below. For payment by credit card, print and complete Credit Card Payment Form and mail with registration form to address below .

Fax-in Registration
Print and complete Registration Form and Credit Card Payment Form and fax to (303) 724-0911.

Payment must be received for the registration to be processed. Registrations and fees must be received no later than October 11, 2002. A registration confirmation will be sent to you once we receive your registration any payment. Purchase order payment cannot be accepted.

Fees
$325: General Participant- Early registration by August 30, 2002
$350: General Participant- Regular registration after August 30, 2002
$200: Student Participant (Must provide copy of student ID)
$100: Saturday only

Presenters receive $100 discount when purchasing a 3-Day Pass for attending the entire conference. This discount is not valid for presenters attending Saturday only. Each registration includes breaks, reception dinner, conference materials, abstract book, and a copy of the conference proceedings on CD-ROM.

Seating is limited. There is no onsite registration; attendance is limited to preregistered participants only.

Cancellation Policy
A refund will be made on written request prior to October 11, 2002. However, a $25 fee will be retained for administrative costs. No refunds will be made after October 11, 2002. Refund requests must be faxed to Julie Giron at the Center for Human Simulation at (303) 724-0911.
