SUPPLEMENTAL  APPLICATION 

The University of Colorado Health Sciences Center Division of Psychology Supplemental Application Form is available in several different file formats.  You may download the form, save it to disk, edit it with your word processor, and print the completed form for inclusion in your application.  Please select the file format best suited for the hardware and software you use.  Help on downloading word processor documents and converting between them is available on the APPIC web site: www.appic.org  Note: UCHSC Supplemental Application files are not currently available for download via FTP.

MS Word for Windows version 6.0 and higher:

supp2006.doc  when prompted, save to disk.

Word Perfect for Windows version 5.x and higher:

supp2006.wpd  when prompted, save to disk.

MS Word for Macintosh and other Word Processors:

supp2006.rtf  when prompted, save to disk.

If nothing else works:

supp2006.txt  save to disk using the SAVE AS... command in your browser's FILE menu then format with your word processor.
