Rotation Requirements
Once you determine which rotations you plan to challenge and which ones you plan to do prospectively, you should start IMMEDIATELY on the following list of items required to be completed before starting a rotation. You can use the Pre-rotation Checklist as a tool to keep track of completed items.
We must receive written documentation that you have completed all of these items at least one month BEFORE starting any rotation in which you may have patient contact. Please send your documentation (as a full and complete packet) to Pamela Duval, in the NTPD office. If your materials are received piece-by-piece we cannot guarantee they will be addressed immediately. Please keep in mind that completing each item is a university-wide requirement of all students on rotations and is not negotiable.
Gathering written documentation of the immunizations is often difficult to do. Consequently, you may be required to get some injections (even if they are a repeat of what you may have received as a child) or in some cases a blood titer will suffice. As you can see, you must plan ahead in getting these submitted. Students have been delayed in starting rotations when documentation of these immunizations is not complete, so don’t let this happen to you.
- Current immunizations. You must show documentation of up-to-date immunization for several vaccinations. See the Required Immunizations. The specific immunizations required are subject to change based on changing CDC recommendations and University policy. To help you in completing these requirements, go to the Pre-rotation Checklist. This document is provided simply as a tool to assist you in locating the proper documentation. It should not be submitted instead of documentation of individual vaccination requirements. All documentation of vaccinations or titers must be signed by a health professional (nurse or physician). Forms signed by the student will not be accepted.
- Current CPR certification. You must show current certification for adults, children, and infant CPR. No online courses accepted.
- HIPAA compliance. The course can be found on Blackboard at https://blackboard.online.edu.
- Enter your student ID number for the "Username" and "PIN" at "Login Here."
- Once you are logged in, select the "UCD Training" tab at the top center. Find the “HIPAA Training Course" course (making certain you do not take the research version or exam).
- Click "Enroll" at the far right, which will re-direct you to the course.
- After you have completed the online module and quiz, please complete the HIPAA Signature Acknowledgement Form to certify the date that you successfully completed the HIPPA online exam. Your typed name qualifies as your official signature on this form. Please e-mail the form as an attachment to Pamela.Duval@uchsc.edu or fax the form to (303) 315-0831.
- Blood borne pathogens. The course can be found on Blackboard at https://blackboard.cuonline.edu.
- Enter your student ID number for the "Username" and "PIN" at "Login Here."
- Once you are logged in, select the "UCD Training" tab at the top center. Find the "BBP Blood Borne Pathogens for Non-Traditional Pharmacy Students."
- Click "Enroll" at the far right, which will re-direct you to the course.
- Your completion of the course will be recorded electronically without further action on your part.
- Criminal background checks. Required of all students enrolled at the School of Pharmacy. All students must complete the UCDHSC Student Background Investigation Consent and Release form and return to the NTPD office along with a check for $65 made payable to UCHSC SOP.
- Residents of Canada must complete the Hire Right Background Check Form. Canadian students must submit BOTH forms and payment of $65USD to our office.
