To schedule an appointment, please call 720-848-1980.
If you need to send documentation to the Spine Center prior to your appointment, please fax the documents to 720-848-2019.
What to Bring to Your Appointment
Please bring the following information with you to your appointment:
- Past medical records related to your diagnosis
- Previous imaging including X-rays, CT scans and MRIs with reports
- Previous electromyogram studies (EMGs)
- Radiation therapy records, if applicable
- Patient Evaluation Form (Adobe Acrobat file)
- Questions for your physicians
Plan to arrive 15 to 20 minutes before your appointment to complete clinic and insurance forms.
Canceling an Appointment
We appreciate 24-hours notice of cancellation.
Payment and Insurance
We accept most insurance plans and we have health plan coordinators who are available to assist all patients with their insurance-related issues. Questions about payment and insurance can be directed to Patient Account Services at 303-724-5066 or 1-800-298-8682. Telephone hours are 8 a.m. to 4:30 p.m. Monday through Friday.
