If an Insured Person incurs Rx claims within the first 6 weeks of enrollment, the Covered Person must pay for the Rx and submit a claim to Express Scripts after the 6th week at:

After 6 weeks, the Insured Person may go to any network pharmacy and pay 25% coinsurance after a separate $50 point-of-service Plan Year Deductible is satisfied the plan has a $1250 maximum benefit.
