The deadline to waive/select the Student Insurance Plan for new Summer 08 starting students is June 15 , 2008, (if you are a continuing student you will be emailed the plan details and selection/waivre form late June for a the enrolment/waiver deadline of 9/8/08./(returning students can only enroll in the summer plan if they have become involuntarily ineligible for coverage under another group health plan) . Waivers must be postmarked or received no later than the deadline. INCOMPLETE OR LATE WAIVERS WILL NOT BE ACCEPTED!
Do I need to carry insurance?
All students enrolled in 5 or more hours in a degree-seeking program are required to have insurance. As a result, all studentstaking 5 or more credit hours are automatically billed for the Student Health Insurance Plan. If you have a comparable insurance, you may qualify to waive the Student Health Insurance Plan. If you are registered for fewer than 5 hours and do not wish to enroll in the Student Health Insurance, you do not need to waive the insurance.
How do I waive the Student Health Insurance?
The Student Health Insurance may not be the best insurance option for you. We have worked hard to create the best plan possible, however, some students already have satisfactory insurance through an employer, spouse, parent or community. In that case, you probably want to waive the Student Health Insurance charges on your bill.
Step 1 - Determine if your plan qualifies to waive
Your insurance plan must meet several requirements in order to waive the Student Health Insurance Plan. If your plan does not meet these requirements, you will be enrolled in the Student Health Insurance Plan. The requirements are:If you are unsure if your plan qualifies to waive the Student Health Insurance, feel free to contact the Student Insurance Office.
Comparable Coverage means that your health plan meets ALL of the following waiver criteria (Short Term Medical, CICP, and Student Select Plans DO NOT qualify):
- The annual deductible must be no more than $1,000 per Plan Year (HSA’s are not allowable); and
- There is no less than a $200,000 per Injury or per Sickness Maximum; and
- Coverage must include comprehensive services (inpatient and outpatient benefits ($10K or more per injury or sickness for outpatient benefits) in the State of Colorado; and
- Coverage must not be for Emergency Services only (Hospital only and accident only plans are not eligible); and
- Coverage must include outpatient mental health benefits of at least 6 visits per year, at a 50% or higher co-insurance level; and
- Coverage must be active throughout the entire academic year
Step 2 - Obtain a copy of your plan's schedule of benefits.
This documentation should be printed in a policy brochure or can be requested from your insurance company Customer Service department. The schedule of benefits must show that your plan meets the requirements to waive the Student Health Insurance Plan listed in Step 1. Incomplete documentation will not be accepted. Even if you have submitted this documentation in the past you must do so every fall. The selection you make in the fall will carry over to the spring term so you will not need to do anything in the spring unless you would like to select the insurance at that point if you have been involuntarily dropped from other group coverage.
Step 3 - Obtain a waiver form
Waiver forms are automatically sent to all students who are enrolled in 5 or more hours for the Fall. Waiver forms are mailed in July. If you do not receive a waiver form by the end of July, go to the Student Insurance Office to pick one up or print the selection/waiver form and return it to the Student Insurance Office.
Step 4 - Complete and mail the waiver form
If you wish to waive the Student Health Insurance you must complete the waiver form and submit a copy of your plan's summary of benefits.
- The from must be filled out completely including your student ID not your SSN. Incomplete forms will be returned.
- Attach a copy of the front and back of your insurance card and the summary of benefits from your insurance company to the form.
- Bring the waiver form to the Student Insurance Office, or mail it to:

Selecting SHI
Although you will be automatically billed for SHI if you do not waive the student insurance it is still best to return the form marked with I CHOOSE TO ENROLL IN THE 2007-2008 INSURANCE PLAN. This will help reduce the amount of time that it will take for you to show eligible in the SHI Plan with AmeriBen and allow them to start paying on claims with less lag time.
